Please note:
  • We require Certificate of Immunization on Form 3231 before your child can begin school. 
  • For the 3 year old class, your child must be 3 years old & fully toilet trained.
  • For the 4 year old class, your child must 4 years old & fully toilet trained. 

This school does not discriminate on the basis of race, color, national ethnic origin, or religion. 

Noah's Ark Preschool is not licensed and is not required to be licensed by the state and carries liability insurance.


Upon completion of this form, please mail a $150.00 non-refundable fee to Noah's Ark, 366 Log Cabin Road, Milledgeville, GA 31061 Attention: Karen Williams  OR drop by the church office.

2 Day Class - $160/Month
3 Day Class - $190/Month
5 Day Class - $240/Month
